Patient Resources

Being well-prepared for your appointment ensures we have all the information to provide the best possible care.  We also want you to have the answers you need to relieve any anxiety, and to feel educated on your symptoms.

If this is your first visit, we want to make your appointment as smooth as possible.  Please arrive 15 minutes early to complete new patient registration forms. For your convenience – you’ll find the forms below to complete at your leisure before arrival.

Plan to bring the following with you:

  • Patient Health Insurance Card – so we may file your claim. You may want to contact your provider in advance to ensure your benefits and eligible providers.
  • List of current medications
  • Driver’s License or Photo ID of Responsible Party
  • Any referral form if required from primary care physician or ER

Co-payment and deductible is required at time of service.  We accept American Express, MasterCard, Visa, Discover, check or cash.

 

Appointments scheduled Monday-Friday 8:30am-11:30am and 1:30pm-4pm. Eastern Shore ENT schedules carefully to ensure we provide ample time with each patient. If for any reason you cannot keep an appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before the scheduled time.
